NURS3006 – EVIDENCE-BASED PRACTICE IN NURSING

Assessment 1: Short Written Response

Rationale
Evidence-based practice constitutes a vital component of delivering high-quality nursing care and is integral to upholding nursing standards. It ensures that nursing interventions are grounded in the best available evidence, thereby improving patient outcomes. The objective of this assessment is to facilitate an enhanced understanding of the significance of evidence-based practice. By integrating research findings into clinical decisions, nurses can ensure that they provide care that is both effective and efficient. This assessment is designed to provide you with an opportunity to apply your proficiencies and expertise in executing the preliminary stages of evidence-based practice. Developing a strong foundation in evidence-based practice is crucial for professional growth and the advancement of nursing care standards.

Task DescriptionThere are two parts to this assessment.

Identifying the importance of evidence-based practice to nursing practice.

Identifying a clinical problem.

Part 1: Identifying the Importance of Evidence-Based Practice to Nursing Practice (approximately 300-350 words)In Part One, you must explain why evidence-based practice is important in nursing practice. Evidence-based practice helps bridge the gap between research and clinical practice, ensuring that nursing interventions are supported by scientific evidence. As part of your answer, you must identify the link between evidence-based practice and the NMBA Registered Nursing Standards. These standards serve as a guideline for nurses to deliver care that is aligned with current best practices, ensuring patient safety and high-quality care. Your answer should be supported by at least two references. These two references do not include the NMBA Registered Nursing Standards. The use of credible sources will strengthen your arguments and demonstrate your ability to engage with academic literature.

Refer to NMBA Registered Nurse Standards for Practice

Your answer must:i. Clearly define evidence-based practice. Providing a definition allows for a clear understanding of its principles and significance in nursing.ii. Clearly outline the benefits of evidence-based practice and why it is important to nursing care. These benefits include improved patient outcomes, enhanced clinical decision-making, and the promotion of high-quality care.iii. Identify why it is important for nurses to learn about and be involved in evidence-based practice projects. Engaging in such projects enables nurses to remain updated on current research, fostering professional development and competence.iv. Be linked to the NMBA Registered Nurse Standards for Practice Nursing. You do not need to write the standards in full. Choose the standard/s that link or support evidence-based practice and identify them numerically (for example, 1.1., 2.6.), then explain more broadly why you feel these standards are linked to evidence-based practice. Understanding these standards ensures that nursing care is both ethical and effective.v. Be supported by at least two references in addition to the reference for the Registered Nurse Standards. Incorporating scholarly sources adds credibility and depth to your discussion.vi. Use academic writing conventions including paragraphs. Well-structured paragraphs contribute to the clarity and readability of your response.

Part 2: Identifying a Clinical Problem (approximately 300-350 words)In Part Two, you must identify a clinical problem from the included scenario titled Scenario 1: Alex Thorne. Recognizing clinical issues is essential for improving patient care and ensuring that nurses address pressing health concerns effectively.

Your answer must:i. Identify the clinical problem. The problem should be clearly articulated, emphasizing its relevance to patient safety and care quality.ii. Explain why the problem exists (for example: knowledge or practice gaps, process inefficiencies, competing priorities, and conflicting systems). Identifying the root cause helps in designing appropriate interventions and solutions.iii. Outline the significance and harm of the problem using the available literature. This must be done using current literature. Citing recent studies strengthens the argument and highlights the urgency of addressing the issue.iv. Use at least two supporting references. These references should be from peer-reviewed sources, ensuring the discussion is well-supported by evidence.v. Use academic writing conventions including paragraphs. Proper formatting ensures readability and coherence.

Scenario 1: Alex ThorneYou have recently begun work as a registered nurse in a small private hospital. Transitioning into a new clinical environment requires adaptability and strong observational skills. You have been handed over the below patient, Alex Thorne. Receiving accurate and thorough handover information is essential for safe patient care.

Alex Thorne is an 87-year-old gentleman who has been living in a residential care facility for the last five years. Older adults in residential care often have complex health conditions requiring vigilant monitoring. He is demonstrating signs and symptoms of delirium due to a urinary tract infection. Delirium in elderly patients can be distressing and significantly impact their overall health status. He is feeling hot and restless and also feels an urgent need to go to the toilet to urinate frequently. Frequent urination and discomfort are classic symptoms of a urinary tract infection that require prompt medical attention. He recognizes his surroundings but does not know where he is. Cognitive impairment due to infection can pose challenges in communication and care provision. He wants to see Heather, his daughter. Providing emotional reassurance can help reduce anxiety and confusion in patients experiencing delirium.

His observations from one hour ago were:

HR – 94 bpm

BP – 110/70

RESP – 24

O2 sats – 94%

Neuro – E(4), V (4) patient confused, M (6) GCS=14

Temp – 38.2°C

These observations are recorded on paper patient notes with no Rapid Response System triggers such as the Between the Flags (NSW) or MEWs (Qld). The absence of a structured response system may lead to delays in recognizing clinical deterioration. You are familiar with the symptoms of SEPSIS and ask the nurse handing over to initiate the Hospital SEPSIS pathway. Early identification and intervention are critical in preventing complications associated with sepsis. The nurse indicates that she is unaware of a SEPSIS pathway and states, “Yeah, we should keep an eye on it, but for now let’s just get him some additional analgesia and hold off calling for an escalation in care, as I don’t really want to bother the consultant. I heard that they are on a very busy list today.” Such reluctance to escalate care can contribute to poor patient outcomes and increased morbidity.

Referencing
You will need to reference using APA 7th edition. Proper citation ensures academic integrity and acknowledges the contributions of researchers. The age of acceptable referencing sources should be no older than 10 years (unless they are a seminal piece of work). Using recent literature ensures the inclusion of up-to-date evidence and practices. Websites should be avoided unless they are related to government documents (e.g., statistics) from government websites and/or websites that specifically relate to your unit. Relying on credible sources strengthens the validity of your argument and enhances the reliability of your work.
