Instructions: Use this template to create a treatment plan. Provide your response to each area in the box below:

I. Identify a list of problems reported to you by the client and/or caregiver(s).
i. The client and her foster mother reported that she consumed alcohol given to her by her mother at the age of 4.
ii. Lindsey and her foster mother reported that she began smoking cigarettes at age four at her mother and stepfather’s home.
iii. The client and her foster mother reported that she started smoking pot at age nine shortly after returning to her mother.
iv. Lindsey and her foster mother reported that she began abuse of cocaine and methamphetamine when she was aged 11 years.
v. Lindsey reported that she smokes pot for medical purposes and to make her function at home and school.
vi. The foster mother reported that she had found Lindsey smoking cigarettes and pot severally.

II. As you are able, identify a provisional primary psychiatric diagnosis you believemay be present and may need to be addressed. (Note: Refer to the DSM-5 for diagnostic criteria for specific problems listed.)

III. Identify the level of care needed to address the presenting problem(s). This could include:
a. Inpatient
b. Residential treatment
c. Partial hospitalization
d. Intensive outpatient counseling
e. Outpatient counseling

The patient should be admitted to the hospital for some time. She should be subjected to relevant tests to detect the presence of depression. After the test results are out, she may be found diagnosed with depression. Lindsey should then be provided with education on appropriate methods of dealing with depression. Professional therapists should provide the most appropriate medication and treatment to her at the time. Therapists should organize outpatient psychotherapy sessions with her to guide her.

IV. Identified strengths: When identifying goals, include strengths that will help the client achieve (s) (e.g., supportive family). Client should help identify strengths. Initially, it may be difficult to help the client identify more than one or two strengths, but as the course of treatment continues, more should become evident.

VI. Explain one treatment intervention you might use in the case you selected and justify the use of the intervention. Next, select a treatment modality individual counseling, group counseling, family counseling, or a combination of these. Support your recommended intervention and modality with evidence from scholarly resources.

The most effective treatment intervention to apply for Lindsey is conducting psychotherapies. The patient should be subjected to therapy sessions where she is guided and counseled about the harmful effects of drug and substance abuse. Psychotherapies are appropriate in this case because the medication may have some side effects on the patient (Unutzer& park, 2012). Lindsey may get used to medications a situation that will lower their effectiveness.
The best treatment modalities to apply are individual counseling and group counseling. The client should be subjected to personal therapy sessions to counsel her on drug use. Sessions with other people of her age are also essential for her to hear and understand how other youths are struggling with drug addiction.

Cultural sensitivity in planning and evaluation involves asking the right questions, selection of appropriate screening and assessment instruments and selection of effective treatment providers and modalities for each client.
VIII. Explain how you would involve the parents/guardians in the treatment plan and why their involvement might be important.

The guardian would be vital in monitoring the progress of the patient while at home. She also would provide guidance and counseling to the patient in their free time. Lindsey’s foster mother would play the role of being a good role model for Lindsey to emulate. It is important to involve the guardian in the treatment plan because she can ensure the patient attends therapy sessions by accompanying her.
